Getting started is easy; please phone or email Dr. Miller!
Include basic information such as your child’s grade level, learning needs, and best times to speak by phone.
To prepare for the initial meeting with Dr. Miller, download, print and complete the four forms on this page. Dr. Miller will advise you of any other documents that will assist us in developing an educational plan for your child.
